Directions for receiving insurance coverage for the Gnosh, Inc products:
This is how you can use medical insurance to purchase or reimburse for the tools:

NOTE: All insurance companies are different. You need to contact your medical insurance carrier to determine the procedure to follow. It takes some time to do this and you will likely wait for them to process your request. You can order and submit the invoice to your insurance AFTER you find out the proper channels to submit your request.

1. Obtain a skilled feeding-swallowing assessment that clearly states the need for this tool with an ICD-10 medical diagnosis such as Dysphagia R13.10 or oral phase dysphagia R13.11.

2. Print out the shipping page displaying the item (s) you want to order. This should include the item and the cost. (submit this as a bill with the other documentation).

3. Obtain a physician's prescription for the item ("Side Biter therapeutic chewing-swallowing exercise device to treat dysphagia") WITH the diagnosis of DYSPHAGIA and the code.
4. Use the purchasing code E1399 for Miscellaneous Durable Medical Equipment
5. The insurance company may ask for more information about the Gnosh, Inc.
